Informed Consent for Therapy
Counseling is a confidential process designed to help you address your concerns, come to a greater understanding of yourself and learn effective personal and interpersonal coping strategies. Counseling provides a relationship between you and me, the therapist. I have the desire and willingness to help you accomplish your individual goals. The process involves sharing sensitive, personal and private information that may at times be distressing. During the course of counseling, if you experience any periods of increased anxiety or confusion, please bring up the same in your sessions.
Confidentiality
All interactions with counseling services, including
- scheduling and attendance of appointments in person or online on Skype or other alternate video forums,
- the content of your sessions,
- progress in counseling and
- your records
are confidential. No information will be used without your prior permission.
Exceptions to Confidentiality

The therapists’ works as a team. I may consult with other counselors / supervisor to provide the best possible care. These consultations are for professional and training purposes. However your identity will not be revealed.

If there is an evidence of clear and imminent danger or harm to self and / or other, a therapist is legally required to report this information to the trusted individuals or authorities responsible for ensuring your safety
